MEMBERSHIP TERMS AND CONDITIONS – Platinum Generation Card

“Platinum Generation Card” refers to the member-exclusive service provided by Sunway Healthcare Group (hereafter called SHG) for Platinum Generation Card holders. The Platinum Generation Card is a privilege card which gives Card Holders the benefits of enjoying discounts, privileges, events from time to time offered by SHG (referred collectively as Benefits). Card Holder means all the holders of a valid Platinum Generation Card. This AGREEMENT is made and agreed to between Card Holder (You) and Sunway Medical Centre Sdn Bhd.

1. SHG shall have absolute right to revise, add any or discontinue these terms and conditions or Benefits at any time at its sole discretion without prior notice.
2. The Platinum Generation Card is non-transferable and non-assignable and is only for the exclusive use of the Card Holder only.
3. You shall provide information as required to register for the Platinum Generation Card and ensure the information submitted is accurate, current and complete. If, on reasonable grounds, the information is suspected to be inaccurate, not current or incomplete, the SHG reserves the right to suspend or terminate, in part or in full, the Benefits. Any direct or indirect expense incurred for reasons aforementioned shall be borne by You instead of the SHG.
4. You acknowledge and agree that any personal data which you provide to SHG in your application for or use of the Platinum Generation Card will be collected, used and stored in accordance with our Privacy Policy available at https://www.sunwaymedicalpenang.com.my/en/privacy-policy/ and applicable law.
5. The Platinum Generation Card does NOT serve as a point accumulation card and does not allow or require Card Holder to collect points for gift exchange or similar purposes.
6. The Platinum Generation Card does NOT serve as a credit card, debit card or any other form of credit service.
7. Discount privileges may be offered only upon presentation of your Platinum Generation Card during registration. There shall be no reimbursement or refund on previous or back-dated transactions if Card Holders fail to present the Platinum Generation Card during registration.
8. Discount privileges offered are at the sole discretion of SHG and shall strictly apply to patient’s that are Self-Payers only. Self-Payers refers to payments directly from your own funds without involving insurance or any other third-party sources.
9. Unless otherwise stated, The Platinum Generation Card cannot be used in conjunction with other cards and/or other promotions. It also cannot be used in conjunction with blanket discounts across the hospitals for the senior category.
10. SHG shall not be liable for any loss or damage incurred by the Card Holders due to any unauthorized use of the Platinum Generation Card.
11. Any request for replacement of Platinum Generation Card for any reason whatsoever including but not limited to loss or damaged cards, shall be subjected to a fee of RM10 per card.
12. Should any of the conditions and privileges are misused or abused, SHG shall be entitled to suspend and terminate the Card Holder’s right to use the Platinum Generation Card without prior notification.
13. Upon successful registration, The Platinum Generation Card’s Benefits may be utilised after a period of 7 working days from the date of successful registration.
14. To qualify for The Platinum Generation Card applicants must be:-
    1. Malaysian citizens or Permanent Residents; and
    2. Aged 60 years and above.
15. For services offered by Sunway Home Healthcare, additional medical supplies not included in their existing packages will be charged separately. Mileage charges will be incurred if service location distance exceeds 20km from a Sunway Healthcare hospital.
16. Any discounts privileges offered for Sunway TCM Centre are not applicable for consultation fees, medication and consumable items.
17. SHG reserves the right to reasonably interpret these terms and conditions if any of them are unclear or equivocal.
18. These Terms and Conditions shall be governed and construed in accordance with the laws of the Malaysia. Any dispute shall be referred to the courts of Malaysia.

20. You represent and warrant that:
    1. you will not apply for more than one Platinum Generation Card;
    2. you will not share your Platinum Generation Card;
    3. you will keep all information provided by you up to date and accurate;
    4. you will not transfer your Platinum Generation Card to any other person without prior permission from SHG; and
    5. you are responsible for the use of your Platinum Generation Card and all activities that are carried out, whether authorised or unauthorised and must immediately notify SHG if you become aware of any unauthorised use of or access to your Platinum Generation Card.
21. All trademarks, service marks, logos and trade names, whether registered or unregistered, are proprietary to SHG or to other companies where so indicated. You may not reproduce, download or otherwise use any such trademarks, service marks, logos or trade names without the prior written consent of the appropriate owner thereof.
22. Each Card Holder assumes all risks and liabilities for loss or damage arising from his or her use of the Platinum Generation Card.
23. Each Card Holder shall indemnify and hold SHG and their respective directors, officers, employees, agents and contractors harmless from and against any and all claims, proceedings, loss, damage, costs and expenses (including legal costs on a full indemnity basis) incurred, suffered or sustained by SHG and their respective directors, officers, employees, agents and contractors directly or indirectly arising from his or her use of the Platinum Generation Card.
